WHY WE COLLECT IT We collect your personal information in order to:
provide you with safe, best-practice osteopathic care.
We require this information to ensure that you are receiving the appropriate care, without it we are unable to proceed with your consultation. We will not ask for or retain information that is outside of the purpose of your healthcare needs. We may occasionally use your contact information to update you on important clinic matters that relate to your care.
HOW WE COLLECT & STORE IT We use Cliniko to gather, record and store your health information and information regarding their commitment to your privacy is available here. We are confident we are doing the best to keep your information safe however should there be a breach you will be notified in a timely manner to discuss the matter.
WHO HAS ACCESS All our staff are contractually obliged to maintain your privacy. Our receptionists input your initial intake form and only your practitioner(s) will access your ongoing treatment notes. If you have an ongoing claim with ACC, we share only the necessary information with them regarding invoicing through a third party, Submitkit. We must also provide your treatment notes for any ACC related consultations as requested by them and agreed by you on submission of an ACC claim.
HOW LONG DO WE KEEP IT We must keep your health information for 10 years. In order to provide you with continuity of care across your lifespan we will retain your notes for as long as you remain a patient of Barrington Osteopathic Clinic . Should you leave the practice or the practice ceases your information will be appropriately erased after 10 years. All paper notes (used at Barrington Osteopathic Clinic prior to 2020) are stored securely and will be destroyed in 2030, unless otherwise directed by you.
YOUR RIGHTS You have the right to ask for a copy of any personal information we hold about you, and to ask for it to be corrected if you think it is wrong.
